Initial Comments:
Based on the findings of an off site follow-up survey conducted October 29, 2021 , Bethlen Communities Home Health services was found to have corrected the deficiencies and to be in compliance with the requirements of 28 Pa. Code, Part IV, Health facilities, Subpart G. Chapter 601. The deficiencies were cited as a result of an unannounced onsite Medicare Recertification and State Re-Licensure survey completed on July 16, 2021.
